Biddeford Cultural Heritage Center Hall of Fame Award 2021  
Nomination Form

The Biddeford Cultural Heritage Center Hall of Fame Award honors outstanding individuals in our community who have achieved success through their talents, perseverance, accomplishments, and work ethic in making Biddeford and the greater world a better place in which to live and work. Individuals may be chosen from any time period, from our country’s first inhabitants to those among us today. Nominees may be teachers, entrepreneurs and innovators, business men and women, historical figures, and city and religious leaders, among other designations. Consideration will also be given to how the nominees’ cultural heritage affected their accomplishments. Chosen nominees will be honored at a City Hall ceremony and have their names on a special Hall of Fame plaque on display at Biddeford City Hall. Nomination Criteria:

1. The nominee must be or have been a citizen of Biddeford at some time in their lives, or may be a nonresident who has had a significant impact in the city.

2. Consideration will be given to how ethnicity played a part in the nominee’s life, or towards their accomplishments.

3. The nominee must have accomplished something of significance or performed something that has made a positive impact in the city and/or the greater world.

4. Testimonials are encouraged as part of the nomination. These can be from family, friends, clergy, employers, city officials, etc. Nominators are encouraged to detail nominees’ lifetime achievements and to solicit testimonials. 5. Nominees must agree to be photographed and participate (if applicable) in the award ceremony at the La Kermesse Festival in Biddeford in June. The recipient, if unavailable, may designate a person to represent them at the presentation ceremony.

6. A person may nominate themselves.

7. After all nominations from the public are received, the Selection Committee shall have the option of adding as many nominees as they see fit. Who would you like to honor? Use the Nomination Form on the reverse. The deadline for nominations to the Selection Committee is May 6th 2021.
